Acid-Base Equilibrium in Obstetrics


Abstract
Acid-base balance during pregnancy, labor and puerperium in the blood of mother, fetus and
new-born, was reviewed by the literature with the following results:
1, pregnancy is accompanied by respiratory alkalosis due to hyperventilation and secondary
latent metabolic acidosis.
2. Both hyperventilation and metabolic acidosis resulting from oxygen deficit are accentuated during labor, the latter is mainly caused by accumulation of lactic acid.
3. As the concentration of maternal progesterone falls abruptly just after delivery, so the hyperventilation returns to normal with the metabolic acidosis gradually normalizing.
4. The acid-base balance of. fetus is altered by the maternal hypoxemia. But fetal tolerance to low oxygen tension is greater than the mother¢¥s.
5. The homeostatic action for the new-born to adapt the acid-base imbalance is so immature that metabolic acidosis incurred by asphyxia might threaten the life of the neonate.
